Facility Evaluation Report
On 7/24/2026, Licensing Program Analyst (LPA) Grace Donato conducted an unannounced Required 1 Year visit and met with Administrator Ariel Palencia and explained the purpose of the visit.
During visit, LPA toured the facility inside and out. LPA toured the kitchen area. LPA observed there to be a perishable food supply of at least two days and a non-perishable food supply of at least seven days. LPA observed there to be locked cabinets for sharp objects and cleaning supplies. Each bathroom had functioning lights and available soap and paper towels. The water temperatures in the bathrooms were 106 F. Each bedroom had working lights and available bedding and clothing storage areas. LPA toured the outdoor areas and found the exits to be clear of obstructions. Emergency drills are being conducted quarterly.
LPA reviewed five resident records and three resident records. All were found to be complete. LPA also reviewed resident medications. Medications are placed in a locked cabinets. All medications have been logged and found to be complete.
