The inspector’s account

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management visit to conduct a Non-Compliance Plan Quarterly Visit and met with Cyril Inneh, Administrator (ADM). LPA announced the purpose of the visit. LPA observed 3 staff including ADM. LPA observed 3 residents in patio.

The purpose of the visit is to ensure the facility is adhering to the Compliance Plan submitted to Community Care Licensing (CCL) after an informal meeting held on 10/23/24.

During visit LPA toured the facility and observed rooms to be clean with new mattress covers (photo taken with state provided phone). LPA observed bed bug traps on the foot of each bed. LPA did observed dead bed bugs in trap in room #8. ADM stated that bed bug trap had been cleaned on 06/20/26. ADM stated the facility is still getting exterminator treatment by third party agency and provided copy of invoice for the treatment that was conducted on 06/22/26 from 3:24 pm to 4:14 pm interior and exterior areas treated. ADM stated he is still doing his own treatment in between the third party agency treatment.

LPA toured 2 resident bathrooms. The water temperature measured with thermometer at 106.1 to 115.6 degrees F. ADM provided a copy of water temperature log the last temperature check was conducted on 6/10/26.

LPA toured the kitchen. LPA observed a closet located in locked office where toxics and cleaning supplies are located.

No deficiencies cited during today's visit. This report was reviewed with Cyril Inneh, Administrator and a copy of the signed report was provided.
