Facility Evaluation Report
On 03/26/26 Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management visit to conduct a Non-Compliance Plan Quarterly Visit and met with Cyril Inneh, Administrator (ADM). LPA announced the purpose of the visit. LPA observed 3 residents on front porch and 1 resident eating breakfast. LPA observed 3 staff including ADM.
The purpose of the visit is to ensure the facility is adhering to the Compliance Plan submitted to Community Care Licensing (CCL) after an informal meeting held on 10/23/24.
During visit LPA toured the facility and bedrooms #1-12. LPA observed bed bug traps on the foot of the beds. LPA obtained pictures with state provided phone. During tour LPA inspected the kitchen cabinets and pantry. LPA did not observe any cockroaches or bed bugs in the kitchen. LPA toured bedroom #11 and found a live ccockroach on residents dresser, LPA was unable to obtain a photo due to resident smashing cockroach.
ADM stated the facility has fewer bed bugs and cockroaches then previous visit on 03/05/26 but continues to see bed bugs and cockroaches. The facility's last exterminator treatment was conducted on 01/12/26. ADM stated the facility did not have a treatment in February and provided a confirmation for next scheduled treatment set to occur on 03/30/26. ADM stated he will provide receipt of treatment to the department.
ADM stated the facility is still conducting their own treatment for bed bugs with powder and spray in between exterminator treatments.
