Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted a unannounced case management visit to follow up on the relocation plan to move residents for the remodel and treatment of bed bugs. LPA met with Cyril Inneh and announced the purpose of the visit.

ADM stated the background check for the 2 tenants that live at the relocation cite 1 has cleared and 1 is pending but is planning on relocating them to another location.

ADM stated he submitted a STD850 with a fee for a new fire clearance for the relocation 373 E. San Fernando St. San Jose CA on 1/13/26 with a sketch of home and a evacuation plan.

ADM stated once the fire clearance is approved will be relocating all 16 residents to relocation cite. ADM stated all POAs and Care Managers are aware of the relocation plan.

An exit interview was conducted with ADM and a copy of report was provided. no deficiencies cited during case management.
