Facility Evaluation Report

Licensing Program Analyst Marcela Yanez conducted an unannounced case management visit and met with Administrator Cyril Inneh, LPA announced the purpose of the visit. LPA Yanez is following up on a case management that was conducted on 12/10/25 and 12/11/25.

During visit LPA toured the facility and observed the facility conducting maintenance and cleaning for bed bugs. LPA observed all bedding was removed in bedrooms #1-12. The facility sprayed for bed bugs and will be replacing bedding on mattresses. LPA observed bedoom #4 to have a hole size measure 4x4 inches, ADM stated the hole will be repaired and send photograph to LPA once repaired.

During tour LPA observed a 2 burner propane stove being used instead of the kitchen stove while the electric stove is installed. ADM stated new electric stove needed a special voltage plug in order to be installed. ADM stated propane burner is being used temporarily to cook meals.

LPA observed furnace to be working properly and room temperature on portable heater on second floor displayed 85 degrees f. LPA observed facility heater duct blowing warm air.

During visit LPA measured the water temperature and it measured with thermometer in 2 resident bathrooms at 107.9 -122.5 degrees F. ADM was reminded the water temperature needs to be between 105 -120 degrees F. ADM stated water is being adjusted after water heater was relit. During visit LPA inspected the stairs hand rails, stairs from front door to street slightly move when forcefully shaken.

No deficiencies were cited as per California Code of Regulations Title 22. This report was reviewed with Administrator Cyril Inneh and a copy of this report was provided.
