Facility Evaluation Report
On 12/10/25 Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management visit to conduct a Non-Compliance Plan Quarterly Visit and met with Cyril Inneh, Administrator (ADM). LPA announced the purpose of the visit. LPA observed 3 residents on front porch.. LPA observed 3 staff including ADM.
The purpose of the visit is to ensure the facility is adhering to the Compliance Plan submitted to Community Care Licensing (CCL) after an informal meeting held on 10/23/24.
During Visit LPA reviewed 3 residents files. and 2 staff files. LPA observed both staff have fingerprint clearance. LPA toured the facility grounds and bedrooms #1-12 and observed the feet of the bed to have bed bug traps with dead bed bugs. ADM stated the facility is undergoing treatment and is planning to do a tenting on the house to treat bed bugs. ADM stated the last treatment from exterminator was conducted on 11/24/25. ADM provided proof of treatment from third party exterminator. ADM stated the facility is still conducting their own treatment for bed bugs with powder and spray in between exterminator treatments.
During tour LPA observed Toxics locked in a closet located in the locked office.
ADM provided a log of hot water temperature for the month of October and November.
No deficiencies were cited as per California Code of Regulations Title 22. This report was reviewed with Administrator Cyril Inneh and a copy of this report was provided.
