The inspector’s account

Licensing Program analyst (LPA) Marcela Yanez conducted an unannounced case management visit. LPA announced the purpose of the visit and met with Cyril Inneh, Administrator.

On 12/10/25 LPA Yanez conducted an unannounced visit. Upon arrival LPA observed a strong gas smell in the facility. ADM called Pacific Gas and Electric. Technician arrived at the facility and measured the facility for gas leak. ADM stated technician turned off all appliances and furnaces as well as water heater and observed meter to show some leakage but it was not life threatening and was not a hazard to residents in care. ADM was advised by PG&E to check the line from the street to the home for any leakage. PG&E technician tightened the valves and gas pipes and determined the facility safe for residents to remain in facility but the gas is turned off until repairs are done. ADM provided a copy of the report PG&E technician provided.

LPA reminded ADM to follow disaster plan for relocating residents, at this time there is no running hot water and no working stove and no working heater since gas is turned off until repairs are done. ADM stated he will begin relocation plan starting 12/10/25. ADM called staff to come and assist with relocation.
