The inspector’s account

On 09/08/2025 Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management visit to conduct a Non-Compliance Plan Quarterly Visit and met with Cyril Inneh, Administrator (ADM). LPA announced the purpose of the visit. LPA observed 5 residents in back patio area and 2 residents in front porch. LPA observed 3 staff including ADM.

The purpose of the visit is to ensure the facility is adhering to the Compliance Plan submitted to Community Care Licensing (CCL) after an informal meeting held on 10/23/24.

During visit LPA toured bedrooms #1-#12 and observed bed bug traps and treatment powder along the edges of the bedrooms. ADM stated the facility will be increasing the exterminator treatment to every month starting the next treatment, ADM will provide copy of treatment once completed. ADM provided treatment receipt from exterminator that shows the facility was treated for bed bugs on the facility perimeter and the bedroom base boards in all bedrooms #1-#12 on July 21,2025 the exterminator receipt states the facility was treated from 10:00 AM-10:40 AM.

The facility conducts its own treatment for bed bugs with powder, last treatment was done on 08/28/25 in bedrooms #7, #8, #9, #10, #11, #12. ADM provided a log with dates and rooms treated with powder and Alpine(Spray)

LPA toured the facility inside and out and observed the facility clean and in good repair. LPA observed toxins stored in the facility office locked an inaccessible to residents in care.
