Licensing Program Analysts (LPAs) Christine Dolores and Steve Chang arrived at the facility unannounced and met with Administrator, Cyril Inneh.
The purpose of the visit was to deliver an immediate exclusion letter to exclude an employee at the facility.
The letter was handed to the Administrator during today's visit.
This report was reviewed with Administrator, Cyril Inneh and a copy of the report provided.
