Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management visit. LPA announced the purpose of the visit. LPA met with Vanessa Carranza.

LPA case management visit is to follow up on the facility converted garage to a 1 bedroom with bathroom and kitchen unit. LPA spoke with Eduardo Carranza via telephone, regarding the facility garage converted to the Additional housing unit. Mr. Carranza was able to verify the facility garage conversion was started in 2018 and completed on 09/03/2019. Mr. Carranza stated that he thought the city of San Jose permit department would notify licensing. LPA reminded Mr. Carranza that before any alterations are made to the structure of the facility the department needs to notified prior to any conversion.

LPA spoke to Administrator Vanessa Carranza regarding the conversion of the garage. ADM stated that any future alterations to the facility she will notify licensing.

A Deficiency was cited as per California Code of Regulations Title 22. This report was reviewed with Vanessa Carranza, Adminsitrator and a copy of this report and appeal rights were provided.
