Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management visit. LPA announced the purpose of the visit to review current fire clearance and the current tenants in the attached unit. LPA met with Lead Staff Mary Lou Bancud.

LPA called ADM Vanessa Carranza via telephone and asked to meet at the facility. ADM stated she is stuck in traffic but discussed the current fire clearance with LPA and the 2 tenants that are living in the attached unit garage conversion that has a separate living area with kitchen, bedroom and bathroom. The converted garage has a separate address from the facility 2565 B Sugar Plum Drive San Jose Ca 95148.

ADM stated that the 2 tenants do not interact with the residents because it is a separate dwelling unit. ADM was unable to provide date of conversion during visit.

ADM stated she will be submitting a facility sketch for the current license to LPA via email and request a fire clearance with CCLD for the current license.

No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with Lead Staff Mary Lou Bancud and a copy of the report was provided.
