Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced 1 year required visit. LPA met with Maintenance Supervisor, Administrator Amy Hellyer arrived at 10:30 AM.

During visit LPA toured the facility inside and out. LPA observed the kitchen area and locked cabinets under sink for toxics. The facility has knives are locked in a lockbox located in the kitchen. LPA observed perishable food supply of at least two days and a non-perishable food supply of at least seven days. The facility refrigerator measured with thermometer at 40 degrees F, and freezer at 0 degrees F. The facility room temperature displayed 73 degrees F. LPA toured 3 resident bedrooms. Each bedroom had available bedding and clothing storage areas as well as functioning lights. LPA toured two out of two resident bathrooms. Each bathroom had available soap and paper towels and functioning lights. The water temperatures in the bathroom sinks measured with thermometer between 106.8 to 107.9 degrees F. LPA observed 1 staff bedroom

Staff tested the smoke detector in the kitchen hallway and smoke detector functioned properly when tested.

LPA toured the outside area and found the exits to be clear of obstructions. LPA observed fire extinguisher was last serviced on 06/24/2025. LPA reviewed the disaster drill log. The facility last fire drill was conducted on 03/11/26 and earthquake drill on 4/13/26. The facility has an a great room for activities.

LPA reviewed resident records for 3 residents, and P&I. LPA reviewed 3 staff records. The facility practices Christian Science Faith and has a waiver for Medical Assessment, examination tests, medical history dated July 27, 2018 for staff and residents.

No deficiency were cited as per California Code of Regulations Title 22. This report was reviewed with Amy Hellyer and a copy of this report was provided.
