Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced case management visit and met with Administrator (ADM) Ini Uboh.

Upon arrival at the facility, LPA notified Administrator to come to the facility. ADM agreed to come to the facility. All residents went to day Program. LPA observed 2 staff (S2, S3) in the facility.

The purpose of today's visit is to deliver a notice to immediately remove staff S1 from the facility and to ensure S1 does not work or present at the facility..

ADM stated he/she received the same notice from US mail before. ADM stated he/she signed the document and submitted it.

Exit interview was provided to ADM. The report was provided to ADM for review and signature. A copy of the report was provided to ADM.
