Licensing Program Analyst (LPA) Steve Chang conducted an unannounced case management visit to deliver the amended investigation finding report and met with Administrator (ADM) Nai Saechao.
LPA addressed the purpose of today's visit to ADM. On 04/14/2026, LPA delivered the investigation finding report. LPA amended the investigation finding report dated 04/14/2026 due to new information obtained.
ADM reviewed the amended investigation finding report and discussed with LPA.
Exit interview was conducted with ADM. The report was provided to ADM for review and for signature. A copy of the report was provided to ADM.
