Facility Evaluation Report

Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Required 1 Year visit and met with Administrator Nai Saechao.

During visit, LPA toured the facility. LPA toured the kitchen area and observed there to be a locked drawer for cleaning supplies.

LPA toured four out of four bathrooms. Each bathroom had available soap and paper towels and working lights. The water temperatures in the bathroom sinks ranged from 115 F to 118 F.

LPA toured the activity rooms of the facility.

LPA obtained a copy of the Inspection Report of the emergency fire detection and sprinkler system. The report states the emergency fire and sprinkler detection system passed its inspection on 11/10/2025.

LPA reviewed the emergency disaster drill log, which indicates the last drill occurred on 11/03/2025.

LPA Marrufo reviewed seven client records and seven staff records. All records were observed to be complete.
