Facility Evaluation Report
Licensing Program Analyst (LPA) conducted a Case Management visit and met with Administrator Nai Saechao (ADM).
LPA explained the purpose of today's visit to ADM. The facility Social Vocational Services, Inc, #435201522, had a NCC meeting on 12/12/2024 for client elopement and SVS San Jose Inclusion Center has the same licensee as Social Vocational Services. The purpose of today's visit is to inspect and review the facility for the care and supervision of clients.
LPA toured the facility with Administrator. LPA checked. All the exit doors have door alarms. LPA discussed the facility policy of care and supervision on clients with ADM. LPA discussed the facility policy of transpiration of clients with ADM. LPA discussed the policy for the inside facility activities and out side facility activities with ADM. LPA checked the physician reports and appraisal needs and service plan of clients with ADM. LPA discussed the facility staffing with ADM.
