Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez & Manuel Monter arrived unannounced to conduct a case management- other to amend a complaint investigation report (LIC9099) due to erroneous information within the report. LPA met with ADM Prince Stanley Amas

LPA met with facility ADM. LPA explained the purpose of the visit.

On April 3, 2025, the Department received a complaint regarding the facility Success RCF1.

On April 11, 2025, the Department conduct an initial investigation and the facility was cited. Due to erroneous information on the report, the complaint investigation was not closed, even though deficiencies were cited. Furthermore an additional allegation was not added to the complaint report.

On May 6, 2025, LPA’s conducted a visit to amend the report. This included finalizing and substantiating 3 of the allegations and continuing the investigations regarding the 2 other allegations.

No deficiencies cited during today's visit. This report was reviewed with Administrator Prince Stanley Amas and a copy of the signed report was provided.
