The inspector’s account

Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced annual required inspection and met with Program Director/Administrator (PD/ADM) Fernando Paz and stated the purpose of the visit.

The facility serves adults 18 and over. LPA observed residents in the backyard, by the receiving area, and some were in their respective rooms.

LPA toured the facility, including common areas, resident rooms, kitchen, bathrooms, driveway, and outdoor spaces and storage areas. Indoor temperature was within acceptable range of 69°F. The kitchen was organized; knives and chemicals were locked. Food supply met requirements for 2 days perishable, 7 days non-perishable. Kitchen water temperature measured at 133°F and a warning sign was posted that the faucet delivers hot water over 125°. Bathroom water temperature ranged from 117.1°F to 117.7°F.

Bathrooms had grab bars and non-skid flooring; resident rooms had adequate storage. Medications were locked and inaccessible to residents. Outdoor areas were free of hazards; laundry appliances were functional. LPA did not observed laundry detergent in the laundry area. Hallways were free from tripping hazards and well-lit.

LPA reviewed 4 client records and 3 staff records, including medication logs, admission agreements, care plans, health screenings, and training. All staff have required clearances and certifications such as 1st Aid/CPR certification.
