Facility Evaluation Report

Licensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced case management visit for deficiencies cited during an annual inspection conducted on 06/16/2026. LPA met with administrator Yahna Dick.

During today's visit (07/15/2026) LPA observed 2 clients and 3 staff. LPA checked the Guardian Website and able to verify that one of the staff cited (S3) for not having a fingerprint and criminal background clearance was under review, the health screening was cleared. However, staff 2 (S2) was not processed by the Care Provider Management Branch (CPMB) had a problem with S2s social security number and is currently being looked into by CPMB and the facility. Staff 4 (S4) fingerprint clearance did not go through and CPMB sent a letter to the applicant's current address. S4 stated he/she did not received the letter in the mail. LPA printed out the form for livescan and the letter that was sent to his/her current address on file. S4 stated that he/she recently came back to work and was not aware that his/her livescan did not go through. Staff 5 (S5) had recently have his fingerprint clearance and is currently being associated to the facility.

ADM stated that employees were fingerprinted and had health screening done prior to working at the facility.

Due to circumstances that requires guidance with CPMB application, no deficiencies were cited during visit. The fingerprints were required by the facility prior to employment, however, the department has different requestor code and livescan needs to completed on the LIC 9163 form.
