Facility Evaluation Report
Licensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced case management - incident inspection and was met by three staff (S1 to S3). The administrator was not at the facility during the time of the visit due to a prior commitment. LPA stated the purpose of the visit to S1 to S3.
The case management - incident report was received by the Department on 05/05/2026 regarding a resident (R1) who was allegedly physically abused by 2 individuals.
LPA Partoza, requested for documents, such but not limited to staff record, training, mandated reporting, staff schedule, resident record, personal rights, appraisal needs and services plan, admission agreement, and physician's report.
At this time, this case in under review and department will conduct a follow up visit , if warranted
No deficiencies were cited during today's visit based on California Code of Regulations (CCR) Title 22 and a copy of the report was provided to care staff Zenaida Villanueva on behalf of the administrator Johann De Guzman.
