Licensing Program Analyst(LPA) conducted an unannounced case management visit to deliver an amended investigation report and and met with Administrator (ADM) Rose Abadam.
Thw investigation finding of complaint with control number 26-AS-20250520102815 is being re-opened due to new information received by the Department.
This complaint allegation is being amended after new information has been received by the Department.
LPA interviewed ADM, staff S1 and resident R1.
The allegations needs further investigation.
Exit interview was conducted with ADM. A copy of the report was provided to ADM.
