Facility Evaluation Report
Licensing Program Analys t(LPA) Marcella Tarin conducted an unannounced annual inspection, and met with Staff S1 April Maimon. S1 stated Licensee/Administrator (ADM) Shirley Emler would not be present for the inspection. S1 states the facility has 3 staff and 6 clients. No clients were present during inspection, all clients were at day program.
LPA toured the interior and exterior of the facility with S1 to include the kitchen, client rooms, dining room, bathrooms, back and front of the facility. This facility has two stories. All exit and passageways were free and clear of obstruction.
LPA toured the kitchen area and observed a perishable food supply of at least two days and a non-perishable food supply of at least seven days. LPA observed the refrigerator temperature at 40 F and Freezer at -1 F.
LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to clients in care.
The facility was equipped with smoke and carbon monoxide detectors. All smoke detectors functioned properly when tested by S1. Fire extinguishers were last serviced on 4/24/2025. The facility emergency drill log was reviewed. The facility's last drill was conducted on 12/17/2025.
During tour of the facility backyard, LPA observed an above ground pond. LPA observed the pond was locked, and inaccessible to clients in care.
