Facility Evaluation Report
Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced required 1 year inspection, and met with Licensee/Administrator (LIC/ADM) Sandy Zipagan.
The facility is licensed to serve six (6) developmentally disabled adults 18 - 59 years old. All of whom may be non-ambulatory. The facility has approved hospice waiver for two (2) residents.
LPA toured the facility inside and out, inspected 3 resident rooms, 1 staff rooms, 1 living room, family room, and kitchen. LPA observed resident rooms have storage for each resident that are sufficient for the residents' personal belongings. Room temperature was 70 degree F. Hot water temperature is at 112 degree F when measured with a digital thermometer.
LPA observed the first aid kit to be complete. Smoke and carbon monoxide detectors functioned properly when tested. 2 Fire extinguishers were serviced on 06/03/2025. Restrooms observed to have non skid flooring. LPA observed 2 days perishable food supply and 7 days non-perishable food supply. Refrigerator temperature observed at 32 degree F and Freezer temperature 0 degree F. LPA observed medication, knives, and cleaning product are stored in a locked cabinet and not accessible to residents.
The front and back yards of the facility were inspected. Exits and walkways were free from obstruction. Facility has an ADU at the back that are used by staff.
