Facility Evaluation Report

Licensing Program Analyst (LPA) Simi Rai conducted an announced case management visit to follow up on an exception request for resident R1 for the use of postural support and half-bed rails. LPA Rai met with Administrator Designee, Marta Malapitan & Lead Staff, Maricel Bamba and stated the purpose of the visit. During visit, LPA Rai observed 3 staff and 6 residents at the facility.

During today's visit, LPA Rai observed 6 out of 6 resident rooms, including R1's room. LPA Rai observed R1 using a full bed rail and chest strap. LPA Rai reviewed R1's facility file and R1 has approved exception for full bed rail and chest strap.

During today's visit, LPA Rai reviewed resident's exception request on file. Administrator Designee, Marta Malapitan has submitted exception requests for resident R2, R3 & R4 to the Department for review.
