Facility Evaluation Report

Licensing Program Analyst (LPA) Simi Rai conducted an announced case management visit to follow up on an exception request for resident R1 for the use of postural support and half-bed rails. LPA Rai met with Administrator Designee, Marta Malapitan & Lead Staff, Maricel Bamba and stated the purpose of the visit. During visit, LPA Rai observed 3 staff and 6 residents at the facility.

During today's visit, LPA Rai observed 6 out of 6 resident rooms, including R1's room. 1 Out of 6 resident beds (R1) was using half-bed rails. LPA Rai reviewed R1's facility file and observed an approved exception for R1 using half-bed rails. 2 Out of 6 residents (R2 &R3) are using wheelchair with seat. LPA Rai reviewed R2's & R3's facility file and observed an approved exception for using wheelchair with seat belt.

During today's visit, LPA Rai reviewed resident's exception request on file and it was determined that this facility does not need additional exception requests at this time and the facility is compliant with California Code of Regulations, Title 22.
