Facility Evaluation Report
Licensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced case management - deficiencies visit and met with administrator Remil Navarro. LPA stated the purpose of the visit to ADM.
On 04/09/2026, LPA Partoza, conducted a required 1-year inspection and cited the facility for the violations under the California Code of Regulations (CCR) Title 22.
80087 Buildings and Grounds (a) The facility shall be clean, safe, sanitary and in good repair at all times for the safety and well-being of clients, employees and visitors.
85068.3 Modifications to Needs and Services Plan (a) The written Needs and Services Plan specified in Section 85068.2 shall be updated as frequently as necessary to ensure its accuracy, and to document significant occurrences that result in changes in the client's physical, mental and/or social functioning.
The citations has been cleared during today's inspection visit 05/07/2026. LPA observed that the facility is kept safe and sanitary. ADM stated that R1 has a tendency to use his/her own dirty clothes to wipe the surfaces of the bathroom and have to constantly follow R1 to ensure that the restroom is wiped dry properly. LPA observed ADM was cleaning the bathroom when LPA arrived at the facility. LPA observed that R2s bedroom floor was organized and sanitary, LPA did not observe food wrappers. Kitchen surfaces were observed to be sanitary and organized, no food was observed under the table next to the refrigerator.
No deficiencies were cited during today's visit based on the California Code of Regulations (CCR) Title 22 and copy of the report was provided and the Letter of Deficiency Citations Cleared was handed over to administrator Remil Navarro.
