Facility Evaluation Report

Licensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced case management visit and was met by staff Marbil Llanes. Staff stated that administrator Thelma Llanes was out of the country since 12/15/2025 and will be back on 12/27/25. The designated administrator is Virgie Quezada and was not available at the time of the visit due to prior commitment.

LPA stated the purpose of the visit was to clear the deficiencies that was cited on 10/24/2025.

LPA was able to speak with the licensee/administrator via zoom and was able to confirm that deficiencies were corrected and addressed by the licensee/administrator and authorized staff Marbil Llanes to sign for the report. LPA reviewed the facility file, resident and staff files. LPA interviewed 2 staff (S1 and S2) who both stated that all deficiencies were corrected the day after LPAs visit and training was given to staff on 10/25/25.

LPA cleared the deficiencies by visit and provided letter of deficiency citations cleared.

No deficiencies were cited during today's visit based on California Code of Regulation (CCR) Title 22. An exit interview was conducted with staff Marbil Llanes and a copy of the report was provided.
