COMPLAINT CONTROL NUMBER: 26-AS-20230818160901
On 12/10/2024, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced complaint investigation visit to deliver findings regarding the allegation received. LPA met with staff Noemi Velasquez and explained te purpose of today's visit.
During the investigation interviews were conducted and facility observations were made. It's discovered through interviews that there has been witnessed accounts of S1 yelling at a resident. Per interviews the main reason for the yelling has been in regards to facility fees and the non-payment of the fees by R1. Based on the interviews the preponderance of evidence standard has been met. This poses a potential health and safety risk to resident in care. This allegation is substantiated.
Based on LPA interviews and items letters received, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 6, are being cited on the attached LIC9099D. Report is reviewed with Noemi Velasquez and a copy of this report is provided on this day.
