Facility Evaluation Report
Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit and met with Administrator (ADM) Shirley Richards.
ADM stated the facility is in a non operational status, and there is no resident in facility.
LPA toured the facility inside and out with ADM. Living room, family room, dining room, kitchen, and garage were inspected. 3 restrooms, 1 office, 4 bedrooms, and 1 master bedroom were observed in the facility.
LPA toured the backyard, no obstruction was observed to block walkway.
No resident was observed in facility or at the backyard.
Exit interview was conducted with ADM. This report was provided to ADM for signature. A copy of the report was provided to ADM.
