Facility Evaluation Report
Licensing Program Analyst (LPA) Steve Chang Conducted an unannounced Plan of Correction visit to follow up the case management conducted on 2/19/2025, and met with Administrator (ADM) Shirley Richards.
The facility was cited as Title 22 section 80078(a) Responsibility for Providing Care and Supervision, and 80087(g) Buildings and Grounds. On 1/16/2025, resident R1 attacked resident R2 with scissors. R2 obtained laceration on the right side of the face and bleeding in the middle head skull. R2 was sent to hospital.
On 2/20/2025, the Department received a plan of correction from ADM.
LPA toured and inspected the facility with ADM. Knives were observed locked. A scissor was observed locked in the office. A scissor was observed locked in the medication closet. A scissor in first aid box was observed locked in the medication closet.
LPA discussed with ADM regarding care and supervision to residents. LPA discussed with ADM regarding the updated appraisal needs and service plan. LPA discussed with ADM about the reporting requirement. LPA discussed with ADM regarding the resident behavior assessment and management. LPA discussed with ADM regarding to lock and store the dangerous items in the facility. LPA discussed with ADM regarding the staff training. LPA reviewed the staff training log.
