Facility Evaluation Report
On 7/20/2026, LPA Grace DOnato conducted an unannounced annual inspection and met with Administrator (ADM) Shirley Richards and explained the purpose of the visit.
LPA toured the interior and exterior of the facility with to include the kitchen, residents rooms, dining room, bathrooms, back and front of the facility. All exit and passageways were free and clear of obstruction. There are currently 4 residents residing in the home and all are out in the day program. LPA toured the kitchen area and observed a perishable food supply of at least two days and a non-perishable food supply of at least seven days.
The facility was equipped with smoke and carbon monoxide detectors. The facility emergency drill log was reviewed. Emergency drills are being conducted monthly.
LPA reviewed four resident records and three staff records. All records are complete and updated. Centrally stored medications was also reviewed. All medications are logged and complete.
No deficiencies cited today. Report is reviewed and copy is provided.
