The purpose of this visit was to amend the Case Management -Incident issued on 11/5/2024 wherein the Department inadvertently did not issue a deficiency on the medication error reported on 10/30/2024.
Exit interview was conducted with House Manager (HM). the report was provided to HM for review. A copy of the report was provided to HM.
