Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Administrator (ADM) Ranulfo Garcia.

LPA reviewed 5 client files and 5 staff files.

License and personal rights poster were observed in the facility. LPA toured the whole facility with ADM.

Lobby, front desk, offices, conference room, activity rooms, arts and craft studio, 5 restrooms, kitchen and storage room were observed and inspected.

Room temperature was 69 degree F. The facility was equipped with smoke and carbon monoxide detectors. Smoke detector alarm system and carbon monoxide detectors were tested, and were working. Fire extinguisher was serviced on 1/6/2026. Fist aid box and emergency light system were observed in the facility.

The last time the facility conducted the fire drill was on 4/2/2026.

Parking lot were observed and inspected. There was no obstruction to block the walkways.

Exit interview was conducted with ADM, No citation was noted today.
