Facility Evaluation Report
On 06/04/25 Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced POC visit. LPA announced the purpose of the visit and met with Administrator/Licensee Lucena Caruz. LPA observed 4 residents and 2 staff.
on 04/04/25 the facility was cited for a Type A violation for 87303 Maintenance and Operations for the facility bathroom # 2 shower faucet to be in disrepair and the hot water temperature measured with thermometer under 105 degrees F.
On 06/04/25 LPA Yanez toured the resident restroom and tested the water with thermometer measured at 118.6 degrees F. LPA also observed shower faucet was replaced with new shower head in restroom #2, and in restroom #1 the light fixture was also replaced with a new light fixture. LPA observed heater wall was covered.
LPA toured the inside and outside of the facility and found exits to be clear and free of obstructions. LPA observed the fountain in backyard was removed.
LPA Yanez cleared the deficiencies cited on 04/04/25 during today's visit. A Letter of Deficiency Citations Cleared was printed and provided to ADM/LIC during today's visit.
No deficiencies cited during today's visit. This report was reviewed with ADM/LIC Lucena Caruz and a copy of the signed report was provided.
