Facility Evaluation Report
Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced annual inspection visit, and met with Administrator Ranielle Manzano. During the visit, LPA observed 0 residents and 1 staff. LPA explained the purpose of the visit. ADM stated the home is a level 2 ARF.
LPA toured the facility inside out with ADM which included the Living room, kitchen, dining room, 4 restrooms and 3 residents bedrooms. The staff area of the facility was also inspected. The front yard and backyard were inspected. There was no obstruction to block the walkways. LPA toured the facility garage, which is being used as a storage area.
While touring resident bedroom 3, LPA observed 2 medications (Tums & Melatonin) on a residents dresser. Resident bedroom #3 is a shared bedroom. ADM secured medications during the visit.
While touring the facility kitchen, LPA requested to see the knives storage area. LPA observed the knives storage area uses a magnet to lock unlock. But when LPA pulled the cabinet door, with minimal force, the door opened. LPA informed ADM the knives storage area can be unlock with a slight pull.
Two-day perishable food supplies and seven day nonperishable food supplies were observed. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. Room temperature was at 72 degrees F, and hot water temperature was measured at 115 degrees F in resident bathrooms.
