Facility Evaluation Report
Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management visit and met with Administrator Cecilia Eustaquio. The purpose of the visit was to follow up on deficiencies cited on 07/17/2026.
During visit, LPA Marrufo observed that the cabinets in the kitchen in the house located at 1992 Park Avenue did not have knobs installed. LPA Marrufo observed that the cabinet in the backyard was unsecured and had cleaning chemicals inside. LPA observed there was a bottle of liquid fertilizer in the backyard. LPA Marrufo observed there to be an unsecured prescription bottle of ointment on top of a drawer in the living room of 1992 Park Avenue.
During visit, LPA Marrufo observed that the black refrigerator in 1992 Park Avenue had a locked chain around it, preventing it from being open. LPA Marrufo observed that the top loading freezer in the dining room in 1998 Park Avenue was locked.
During visit, LPA Marrufo interviewed Administrator Cecilia Eustaquio. During interview, she stated that snacks are left out after 5:00 PM and residents can ask for snacks at any time.
During visit, LPA interviewed residents R1-R6, who stated the facility provides snacks to residents after 5:00 PM. R4 and R5 stated that they can ask the kitchen staff for snacks at any time of the day to receive snacks. R1, R2, R3, and R6 stated snacks are only available to residents after 5:00 PM.
