The inspector’s account

On April 17, 2026, Licensing Program Analyst Manuel Monter conducted a case management – incident visit to follow up regarding an incident regarding resident R1. LPA met with Administrator (ADM) Naward Smith and explained the purpose of the visit.

On April 3, 2026, The Department received an incident report regarding Resident R1. The incident report stated, On April 2, 2026, at approximately 11:30 a.m., R1 was eating lunch at PACE Matranga House with staff assistance when he/she began experiencing difficulty breathing. Staff immediately encouraged him/her to remove the food from his/her mouth and initiated the Heimlich Maneuver. After sitting down, R1 began to lose consciousness and slowly leaned toward the floor. Staff supported his/her descent, ensuring he/she was safely positioned on the floor. Once on the floor, staff initiated CPR and continued until emergency medical services arrived.

During the course of the investigation, the Department uncovered technical violations regarding the facility's video surveillance. LPA discussed with Administrator Naward Smith the use of video surveillance in the facility and ensuring these tools are not infringing on residents personal rights. ADM Naward Smith agreed and understood.

LPA provided reference material 2-5800 Guidelines for video surveillance.

A technical violation was cited during todays visit. This report was reviewed with Administrator Naward Smith and a signed copy of the report was provided.
