Facility Evaluation Report
On April 7, 2026, Licensing Program Analyst Manuel Monter conducted a case management – incident visit to follow up regarding an incident regarding resident R1. LPA met with Administrator (ADM) Naward Smith and explained the purpose of the visit.
On April 3, 2026, The Department received an incident report regarding Resident R1. The incident report stated, On April 2, 2026, at approximately 11:30 a.m., R1 was eating lunch at PACE Matranga House with staff assistance when he/she began experiencing difficulty breathing. Staff immediately encouraged him/her to remove the food from his/her mouth and initiated the Heimlich Maneuver while another staff member called 911. R1 remained alert and responsive during this time. He/She was able to take a sip of water and began walking toward the couch. Staff closely monitored and followed him/her. After sitting down, R1 began to lose consciousness and slowly leaned toward the floor. Staff supported his/her descent, ensuring he/she was safely positioned on the floor. Once on the floor, staff initiated CPR and continued until emergency medical services arrived. R1 was transported by 911 to VMC, where he/she is currently admitted to the ICU for further medical evaluation.
On April 7, 2026, Licensing Program Analyst Manuel Monter interviewed Administrator Naward Smith. ADM stated regarding the incident that R1 does, not require 1 on 1 care. ADM stated R1 is currently still at the hospital.
