Facility Evaluation Report

Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced Plan of Correction (POC) visit and met with designated administrator (DADM) Edjul Salvador

Administrator Carlito Ocampo was unable to meet with LPA due to prior commitment.

During POC visit, LPA verified the water temperature based on the written plan of action that facility has provided to CCLD on 12/23/2025.

LPA advised ADM to continue to monitor water temperature to conform with regulations.

ADM stated that the water temperature is monitored by staff and are logged regularly.

ADM will continue to have the water temperature monitored for the safety of residents in care.

POC reviewed and cleared the Type A deficiency cited on 12/23/2025 under California Code of Regulation (CCR) Title 22; 80088 (e)(1) on site and provided a copy of POC Clearance Letter to designated administrator Edjul Salvador

No deficiency cited during today's inspection. Exit interview conducted with administrator. A copy of the report was provided.
