Facility Evaluation Report
Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced Required 1 Year visit and met with Administrator (ADM) Carlito Ocampo and stated the purpose of the visit.
The facility serves 18 to 59 years old who are developmentally challenged, 6 ambulatory, 6 out of 6 may be non-ambulatory. No resident were present during the time of the visit, all residents were attending day program.
LPA inspected the facility inside and outside and observed the exits were clear of obstruction. Inspected 3 resident rooms, 1 staff room, 2 bathrooms, 2 living area, kitchen and garage. LPA Partoza observed food supply of 2 days of perishable foods and 7 days of non-perishable foods. LPA Partoza observed locked cabinets for chemicals, medications and sharp objects and are not easily accessible to residents in care.
LPA observed the washer and dryer are in good working, chemicals are separated from the food pantry. LPA observed 3 out of 3 resident room were sanitary and organized and have sufficient storage for residents' personal belongings. 2 Out 2 bathroom used by residents have grab bars and anti-skid floor mats. Hallways and walkways inside the facility are well lit, free from any tripping hazard and free from obstructions.
LPA Partoza measured the hot water temperature for bathroom and kitchen. The water temperature was measured with a digital thermometer at 136.7 to 141.6 degree F. The facility room temperature is at 66 degree F. LPA observed refrigerator, freezer combination in the kitchen, 1 upright freezer in the garage Refrigerator temperature measured at 37 degree F and freezer is at 0 degree F
