Facility Evaluation Report
Licensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced annual continuation of the required year inspection and met with Executive Director (ED) Minnie Lacson-Weber and Assisted Living Director (ALD) Anomie De Los Reyes and stated the purpose of the visit.
LPA conducted a review of the 6 resident's centrally stored medication record and destruction record (CSMDR). LPA observed that the record was up to date, medications are labeled and matches the physical medication. During the time of inspection and review, LPA observed that record of the medication needs to be readable, organized and located in a timely manner. LPA suggested to ED and ALD to avail of the Department's Technical Support Program (TSP) for record maintenance.
LPA verified that the facility conducted fire drill training on the following dates 05/14/2025 and 07/03/2025. ED stated that the earthquake drill will be provided on their next all meeting scheduled on 09/24/2025.
No deficiencies were cited during today's visit based on the California Code of Regulations (CCR) Title 22. An exit interview was conducted with Executive Director (ED) Minnie Lacson-Weber and a copy of the report was provided.
This concludes the annual required-inspection.
