The inspector’s account

Licensing Program Analyst (LPA) Christine Dolores arrived unannounced to conduct a case management – incident visit. LPA met with Assistant Executive Director, Paula Spanek.

On 08/03/2023 the Department received an incident report and death report for resident (R1).

During visit, LPA interviewed the (AED). The following documents were obtained to include the resident’s physician’s report, functional capabilities assessment, individualized care plan, face sheet, progress notes, medical records, police case number, and verification of death.

This case management will be kept open pending additional information.

No deficiencies were cited per California Code of Regulations, Title 22. This report was reviewed with Assistant Executive Director, Paula Spanek and a copy of the report was provided.
