Facility Evaluation Report
Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced annual inspection visit, and met with Administrator Enrico Aguas. During the visit, LPA observed 0 residents and 1 staff. LPA explained the purpose of the visit. Newlife Care Services, is a level 5 home.
LPA toured the facility inside out with ADM which included the Living room, kitchen, dining room, 2 restrooms and 3 residents bedrooms. The staff area of the facility was also inspected. The front yard and backyard were inspected. LPA toured the facility garage, which is being used a storage area and laundry area.
While touring resident bedroom, which contains a sliding glass door, LPA observed a wooden stick obstructing the path of the sliding glass door. ADM stated the stick was placed there because resident R1. ADM stated R1's family member reported to him, that R1 will wander away as a behavior. While touring the facility family room, LPA observed another sliding glass door. LPA also observed an obstruction, blocking the path of the sliding glass door. ADM stated the obstruction was placed there as well due to R1's potential wandering behavior.
Two-day perishable food supplies and seven day nonperishable food supplies were observed. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. Room temperature was at 65 degrees F, and hot water temperature was measured at 116 degrees F in resident bathrooms.
