The inspector’s account

Licensing Program Analyst (LPA) Marcella conducted an unannounced annual inspection and met with Administrator (ADM) Darlington Odjegbu. LPA stated the purpose of the visit. LPA observed 2 clients and 2 staff at the facility.

LPA toured the interior and exterior of the facility with ADM to include but not limited to the living room, client rooms, dining area, kitchen and garage. LPA observed all exits and passageways were free and clear of obstruction. There are 2 client bathrooms, 1 office, and 3 clients rooms in facility. LPA observed two days perishable foods and seven day nonperishable foods. LPA observed the refrigerator temperature at 40 F and freezer at 0 F. LPA measured hot water temperature at 109.5F in 2 client bathrooms. LPA observed the medication cabinet, sharps and toxics were locked and inaccessible to clients in care. The facility was equipped with smoke and carbon monoxide detectors. The smoke detectors and carbon monoxide detectors functioned properly when tested by the AM. Fire extinguishers was serviced on 6/30/2025. The facility's last emergency drill was conducted on 11/17/2025.

LPA reviewed 2 client files, P & I records, and Centrally Stored Medication and Destruction Records (CSMDRs).

No deficiencies were cited during today's visit, per California Code of Regulations, Title 22. An exit interview was conducted with ADM and a copy of this report was provided.
