Facility Evaluation Report
Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced annual inspection visit, and met with Staff S1, Julieta Saliba. S1 contacted ADM Muna Qaddura and informed ADM the purpose of the visit . During the visit, LPA observed 0 residents and 3 staff.
LPA toured the facility inside out with S1 which included the Living room, kitchen, dining room, 2 restrooms and 3 residents bedrooms. There was no obstruction to block the walkways. The staff area of the facility was also inspected. The front yard and backyard were inspected.
While touring the backyard, LPA observed a deck with a hot tub. LPA observed a wooden wall perpendicular to the hot tub and clients room. This wall had an opening large enough for LPA to walk thru. As LPA went thru the opening, LPA observed a shed, which was unlocked. As LPA pulled the shed door open, the front frame of the shed came loose. Inside the shed was a dresser, cabinet and other items.
LPA continued touring the backyard and observed the fencing behind the hot tub was leaning backwards. ADM stated this occurred sometime last year after a storm. ADM stated she is working with the neighbor to address the leaning shed. LPA also observed a rust like substance on top of the cover of the hot tub.
Two-day perishable food supplies and seven day nonperishable food supplies were observed. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. Room temperature was at 70 degrees F, and hot water temperature was measured at 118 degrees F in resident bathrooms.
