Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Required 1 Year visit and met with Portia Oviso, Administrator. LPA announced the purpose of the visit. LPA observed 5 residents watching television and 1 resident in the dining room.
During visit, LPA toured the facility inside and out. LPA toured the garage area and observed food storage areas and locked cabinets for cleaning supplies and knives. LPA observed the kitchen area and a seperate office with cabinet for medication. LPA observed perishable food supply of at least two days and non-perishable food supply of at least seven days. Refrigerator Temp displayed 33 degrees f and 0 degrees F for freezer.
LPA toured 4 resident bedrooms. Each bedroom had available bedding and clothing storage areas as well as functioning lights. ADM tested the smoke detector in the hallway and found smoke detector to function properly when tested. LPA toured 1 full and 1 half resident bathrooms. Full bathroom had available soap and paper towels and functioning lights. The water temperature in the bathroom sink measured with thermometer at 106.8 F.
LPA toured the outside area and found the exits to be clear of obstructions. LPA observed fire extinguisher was last purchased on 10/26/25. LPA reviewed Fire and Earthquake log, last fire drill on 01/14/26 and earthquake drill on 01/27/26. LPA reviewed 3 resident records. LPA reviewed 3 staff records. LPA and ADM reviewed centrally stored medication and P&I for 3 residents.
No deficiency were cited as per California Code of Regulations Title 22. This report was reviewed with Portia Oviso, ADM and a copy of this report was provided.
