The inspector’s account

Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to conduct the facility's pre-licensing visit for a change of ownership (CHOW). LPA met with Licensee Zenebesh Ghebresellasie. LPA stated the purpose of the visit.

During inspection, LPA observed damage flooring located at the kitchen sink floor underneath a kitchen mat. During inspection of bedrooms, LPA observed in Bedroom #4, a damaged window sill (window sill has exposed nails), a damaged closet door track, and broken window blinds. LPA observed in Bedroom #5 broken blinds, a damaged window screen with holes (window near the bed, facing front of facility), and a damaged closet door track. LPA also observed 4 loose, not secure, wooden hand rails at the kitchen sliding door exit and Bedroom #1 sliding door exit, which lead toward the backyard. The kitchen screen door was also observed with a hole near the handle. Current licensee needs to address the damaged areas noted and ensure that the physical plant is in good repair with no damaged, loose or broken areas prior to issuing license to new owner.

A deficiency was issued per California Code of Regulations Title 22. See LIC809D. An exit interview was conducted with Licensee Zenebesh Ghebresellasie and a copy of this report was provided and appeal rights were provided.
