Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced 1 year required visit. LPA announced the purpose of the visit and met with Dennis Miclat Administrator (ADM). ADM stated there are 164 clients and 78 staff.

During Visit LPA toured the facility inside and out. LPA toured the main activity area where there are tables for resident to do crafts, areas to do exercise, work stations for crafts, paint by numbers, Exercise machine area with treadmills, video dance exercise. LPA observed a theater room with seating and a big screen television. ADM stated the facility has a kitchen for cooking demonstrations.

The facility has 4 refrigerators. The refrigerator temperature measured with thermometer displayed 37-40 degrees F. and freezer temperature measured at 0 degrees F. LPA inspected 3 out of 6 client bathrooms. Each bathroom had available soap, paper towels. Bathroom sinks water temperature measured with thermometer at 109-110 degrees F.

LPA observed exits to be clear of obstructions. The facility has a dual fire alarm system with carbon monoxide detectors and a pull fire alarm that was last serviced on 11/17/2025. LPA observed 5 fire extinguishers last serviced on 5/5/2026. LPA reviewed fire and earthquake log, Last fire drill was last conducted on 05/05/26 and earthquake drill on 04/08/2026.
