Facility Evaluation Report
Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Program Director (PD) Raul Regencia . Facility license and personal right posters were observed in the facility.
LPA reviewed 6 client files and 6 staff files.
LPA toured the facility with PD including lobby area, the 3 big activity rooms, 4 class rooms, movie room, shower room, 4 restrooms, kitchen, break room, offices, conference room, storage rooms and 6 exits. The outside of the facility was inspected. There was no obstruction to block the walkways.
Room temperature was at 70 degree F. The temperature of the freezer was at 0 degree F, and the temperature of the refrigerator was at 20 degree F. Hot water temperature was observed at 115 degree F. First aid box and flash lights were observed at the facility.
The medication storage area, and cleaning product storage area were observed locked and inaccessible to clients. The facility was equipped with smoke and carbon monoxide detectors. Carbon monoxide detectors were tested by staff and were functional. Fire extinguisher was serviced in 11/10/2025. The facility's last fire drill was on 3/19/2026.
