Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Program Director (PD) Raul Regencia and Assistant Program Director (APD) Marion Cozine.

During LPA's visit, APD stated there are 117 clients and 54 staff in the facility.

LPA toured the facility with APD including lobby area, the Great room, 4 activity rooms, movie room, shower room, 4 restrooms, kitchen, break room, offices, conference room, storage rooms and 6 exits. The outside of the facility was inspected. There was no obstruction to block the walkways. Room temperature was at 70 degree F. The temperature of the freezer was at -10 degree F, and the temperature of the refrigerator was at 40 degree F. Hot water temperature was observed at 115 degree F. First aid box and flash lights were observed at the facility.

The medication storage area, and cleaning product storage area were observed locked and inaccessible to clients in care. The facility was equipped with smoke and carbon monoxide detectors. Carbon monoxide detectors were tested by staff and were working. Fire extinguisher was serviced in October 28, 2024. The facility's last drill was on March 6, 2025. .
